Primary Mental Health Care Services for People with Severe Mental Illness

Clinical coordination and support

People with severe and complex diagnosed mental health disorders may need support to coordinate the clinical services to best help them reach their goals.

Community Mental Health Clinical Care and Coordination provides clinical nursing services for patients with severe mental disorders. Clinical nurses coordinate care, planning and review with GPs, psychiatrists and psychologists.

A coordinated and collaborative approach is designed to provide the best possible outcomes for the person with severe mental illness through evidence-based care from mental health professionals.
