How do I refer someone for care?

  • If you know someone who is 65 or older and would benefit from assistance at home, submit or complete the Aged Care Support Referral form.

  • You can submit a referral via the NDIS ECA QLD Referral form for a child aged 0-6 with a disability or developmental delay in the Local Government Areas of Logan City, Redland City, Brisbane City or Moreton bay in QLD, Australia.

  • If you are a carer, or someone you know is a carer of an individual who has a disability, a life limiting health/medical condition, mental illness or is frail and aged you can submit the Carer Gateway Support Referral Form.

  • The Early Childhood Coordinator (ECC) assists families with children aged 0-8 who are facing challenges in accessing early childhood care and education.

  • Rural Youth Mental Health service supports young people aged between 12 and 25 years who are experiencing mental health challenges like early warning signs of anxiety or depression, or are concerned about their mental health.
