How do I refer someone for care?

If you know someone who is 65 or older and would benefit from assistance at home, please complete the Aged Care Support Referral form.
If you know someone who is under 65 and requires disability support services, please complete the Disability Support Services Referral form.
You can submit a referral via the NDIS ECEI QLD Referral form for a child aged 0-6 with a disability or developmental delay in the Local Government Areas of Logan City, Redland City, Brisbane City or Moreton bay in QLD, Australia.
If you are a carer, or someone you know is a carer of an individual who has a disability, a life limiting health/medical condition, mental illness or is frail and aged you can submit the Carer Gateway Support Referral Form

You can complete the form online or download the PDF version* and email the completed PDF form to customercare@benevolent.org.au.
