We do this by collaborating with you to identify and work towards your individual goals. Who is this program for? Supporting Recovery Local Care Team South Western Sydney is for adults, children and young people in South Western Sydney who have experienced family, domestic and sexual violence and are seeking support to heal and recover from these experiences. The program is open to all members of our community.
What can you expect from Supporting Recovery Local Care Team South Western Sydney?Our caseworkers can provide long-termsupport. After we have received a referral, we will contact you to discuss moreabout your circumstances. If you decide to proceed with the program, we will meetwith you for an assessment where you can choose to share your experiences withus and can tell us more about what your hopes are and how we can best supportyou. What can Case Management assist with? Case management support can include assisting you with: Parenting support related to domestic, family & sexual violence Healthy relationships Housing assistance Financial support Legal aid Other supports identified by you as important to your overall health & wellbeing Referral to Supporting Recovery Mental Health Therapy The dedicated Supporting Recovery Mental Health Therapy team includes experienced and trained therapists such as counsellors, psychologists and social workers who specialise in providing trauma-informed and client-centered mental health care. The therapies offered are holistic, culturally appropriate and will support your safety, choice and control.
How can I access Supporting Recovery Local Care Team South Western Sydney Case Management? Accessing the program is easy! If you are already working with another service, they can complete a referral on your behalf with your consent or you can self-refer by clicking on one of the buttons above! You do not need a referral from your GP to access this program.
