The End of Life Pathway (from My Aged Care) provides additional support of up to $25,000 in funding over 3 months – to help you or someone you love to remain at home when time matters most.
If you or your loved one has been advised that there may be less than 3 months to live, this funding can make it possible to stay in your own home, the place that provides you with the most comfort.
Everyone’s wishes are unique – and the End of Life Pathway is designed to reflect that, built entirely around you.
The types of support available include;
- Nursing and Palliative care to manage comfort, symptoms and changing needs.
- Personal care helps you feel clean and comfortable.
- Emotional and social support for you and your family to support your wellbeing.
- Essential equipment such as beds, wheelchairs, or mobility aids to make daily life easier.
- Support for carers, providing practical help and respite.
