Aged Care Services

From everyday activities in the home to staying active, we’re here to help with services and referrals.

We’re committed to helping you stay independent, in your home for longer. We understand that age is no longer a barrier to living an active and independent life, but sometimes support is needed to make the most of every opportunity.

Our team can help you navigate all of the services and government funding available, so that you can make informed choices about what is best for you and your family, including free assistance with your My Aged Care assessment.

You may be eligible for subsidised funding or you can access and pay for services privately. There are 5 types of funding models available to access our Aged Care Services;

  • Commonwealth Home Support Programme (CHSP)
  • Home Support Packages (HSP)
  • Short Term Restorative Care (STRC)
  • Care finder services
  • Private Funding

For further information or to make a referral, please submit the Aged Care Support referral form online or you can contact us on 1800 236 762.
